Immunization Requirements for K-5 School Enrollment

• 5 DPT (diphtheria, pertussis, tetanus)
• 4 OPV or IPV (polio vaccine)
• 2 MMR (measles, mumps, rubella)
• 1 Varicella(chicken pox) or proof of disease
